RN Case Manager
StartDate: 1/20/2025 Pay Rate: $2000.00 - $2700.00
POSITION SUMMARY – RN Case Manager
POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans
MINIMUM REQUIRED QUALIFICATIONS – FL/compact RN license; 3-5yrs acute care exp; 2+yrs hospital case management experience
PREFERRED QUALIFICATIONS – Bachelors degree in Nursing or health-related field
LENGTH OF ASSIGNMENT – 13 weeks
SHIFT / HOURS PER WEEK – 8a-4:30p M-F / 40hrs per week
SYSTEMS – Epic
START DATE – 1/6/2025
